Project: Fedex WCAA Expansion

Customer: Shaw Electric

 

STATEMENT OF PROBLEM:

In May 2023, CCC had excavated and Shaw Electric had installed a single 4" temporary feed conduit to power trailer city.

On January 18, 2024, CCC was excavating in the same area as the temporary feed and hit the 4" temporary feed conduit to power trailer city.  This conduit also fed a camera at the front entrance to the facility (WCAA property) that also went out of power.

 

ROOT CAUSE:

Miss Dig was called in September 2023.  The work took place on January 18, 2024.  Miss Dig was outside the 21 day marking window.  CCC, Shaw Electric, and Christman did not show this temporary feed on As-Builts.  CCC did not follow Christman "Excavation Permit" process 72 hours prior to starting excavation.

 

COURSE OF ACTION:

Do to other issues also, the superintendent was asked and escorted off-site.  WCAA and Christman would not allow CCC to self-perform any future work on-site, but let CCC still hold the subcontract.  CCC subcontracted the balance of the work to another excavation/concrete company.

 

LESSON FOR FUTURE PROJECTS:

Procedures are put in place for reasons. 

Follow the on-site protocol for your work classification ex. Excavation Permits.  

Utilize ground penetrating radar to locate existing utilities within your work area.  

Call MISS Dig if not on private property.  Allow for 72 hours to clear.  Make sure all utilities have responded back, if not put in a 2nd request after the 72 hours. 

Keep a current set of as-builds for CCC and the owner.  Update daily.
