Shaft Wall Mistake / Failed Inspection

  STATEMENT OF PROBLEM: Part of our scope for the Relocation of the Analytical Lab to WRRF was to create a shaft from the roof to the second floor using existing dead space. This involved removing the floor on the 3rd and 4th floor to create the shaft. The shaft was roughly 5'' x 9'' and was used to bring the new supply and return duct work from the roof to the second floor. CCC installed the core studs, core board and double layer drywall to achieve our fire rating. CCC framed in the fire damper incorrectly (the framing for fire damper did not match what the damper manufacturer recommended) and also installed the second layer of drywall prior to City of Detroit Inspection. Failed inspection required us to remove and rehang/re-frame the area correctly.

COURSE OF ACTION: CCC removed the areas that did not pass by the inspector and reworked them. This had to be expedited so other crews could perform their work after CCC was complete.

This resulted in a loss of approximately $46,000

LESSON FOR FUTURE PROJECTS: CCC team should be sure that area is complete per drawings and code prior to calling for inspection. When a Superintendent is pulling double duty and is acting as both Foreman and Superintendent, he should not take the crews "word for it" and ensure himself that everything is correct. Also prior to framing in dampers, be sure that it is to the correct specifications. This should be shared with other Interior Crews in the same situation so they do not repeat the same mistake.
