GM Pontiac Tooling Cart Trenches Dimensional Error

STATEMENT OF PROBLEM
During the revisions being made to the Rotem drawings, on revision 5 Rotem changed the width and depth for one of the two tooling cart rails for each tooling cart rail system located between the presses (4 sets of rails total). The trench for the rail became wider by several millimeters and deeper by several millimeters. By the time this change occured in Rotem drawings,the tooling cart rail pockets had already been designed and issued by IBI and IBI failed to incorporate this change into a revision to the construction drawings. The dimensional error was discovered by the GM press installation team after slab was poured. The discrepancy was able to be worked around by GM press installation team and the press installer.

COURSE OF ACTION
None

LESSON LEARNED
1) Need better oversight from the AE.
2) Need the press supplier do a cross-check of the foundation drawings to ensure that critical items are correct.
3) Most importantly, there needs to be a cutoff date to process drawings. The process drawings were supposed to be completed in October 2015. There were 10 revisions to the process drawings and revision 10 was issued May of 2016. Constant revisions delayed IBI completing design by constantly having to go through revised Rotem drawings.
