STATEMENT OF PROBLEM
Shift turn overs were not formalized, it seemed like it was a bit of a free for all.
ROOT CAUSE
Did not outline a standard shift turnover meeting/huddle which impacted communications from shift to shift.
COURSE OF ACTION
No action was taken as this was recognized in hindsight.
LESSON FOR FUTURE PROJECTS
Need to have formal shift turn over meetings. 1) turn over discussion with entire team lead by PM. 2) individual turn overs by zone leaders at each zone so they can visualize issues when discussing.
