Did not have enough management staff at start of project. The scope and scale of the project was foreign to everyone involved. While we felt we had the appropriate level of resources were woefully short.
As such, expectations of field personnel without this support were beyond reason. This created an environment conducive for fatigue to negatively impact the personnel [demoralization] and the job [management of subcontractors'' execution of scope], as well as invite opportunity for new hazards and potential for injuries and property damage.
