GM Arlington Assembly Non-Conforming Work Poor Concrete Finish

  STATEMENT OF PROBLEM
Poor finish on exterior concrete. Exterior concrete batch error. There was clumping or balling of synthetic fibers on the interior slab. Post project review shows poor grading and compaction of sub-grade and excessive fibers protruding from the slab.

COURSE OF ACTION
Offered GM an extended warranty on the slab.

LESSON LEARNED
Any concrete pour of 60 yd should have a QC representative from the plant present on site. Include the fiber supplier in the prepour meeting. Confirm dosing procedure with supplier. Have better leadership and communication amongst the CCC staff. The decision to stop or proceed should be made with all parties involved. Don''''''''t be afraid to stop the pour.
