DEMO CONTRACTOR FAILED TO IDENTIFY PLANT UTILITIES (FIRE PROTECTION)

STATEMENT OF PROBLEM:
On July 21, 2026, an NADC laborer was torch-cutting a pipe hanger from an aerial lift. During the cutting operation, sparks came into contact with the heat-sensitive element of a fire sprinkler, causing the sprinkler head to activate and discharge. A bus duct was located directly below the activated sprinkler. The fire protection in the building is painted white in the truss space which does make it more difficult to identify.
ROOT CAUSE:
Failed to protect nearby utiltilites from torch cutting activity.
COURSE OF ACTION:
Subcontractor notified CCC and GM Rep of Incident. Fire System and Buss Duct were turned off. The residual water from discharging fire system was cleaned up and the sprinkler head replaced and system recharged. The subcontractor now requires the installation of FR welding blankets or shields to protect nearby fire sprinkler heads and other vulnerable equipment whenever hot work is performed within the potential spark or heat exposure zone. A safety stand-down was conducted with all personnel emphasizing hazard recognition, spark direction during burning, protection of fire sprinkler system and other sensitive equipment, ground personnel duties and responsibilities, and stop work authority. Ground personnel were issued laser pointers to enhance hazard identification and facilitate clear communication with aerial lift operators by accurately indicating potential hazards from the ground. Revised the FMEA for the work task to include more comprehensive hazard controls, clearly defined expectations for overhead trim work, and enhanced responsibilities for both aerial lift operators and ground personnel.
LESSON FOR FUTURE PROJECTS:
Ensure the team performs a walk to help identify overhead hazard prior to performing work.
