STATEMENT OF PROBLEM:

Boilermakers welding floor plates inside the elevated PEMBs (Penthouses on HRSG units) caused an incident where the underfloor spray-foam insulation caught fire. This led to an insurance claim where the insulation had to be blasted off and was replaced with fire-resistant insulation.

 

COURSE OF ACTION:

Insulation had to be removed and replaced.

 

ROOT CAUSE:

Insulation was flammable and welding protection was not considered.

 

LESSON FOR FUTURE PROJECTS:

Ensure that any below-floor insulation for structures where floor plates are to be welded is fire-resistant and is installed after all floor welding complete.
