STATEMENT OF PROBLEM

1. Unable to locate specific material - Avaco did not have a contents list of what was in each skid this resulted in:
     a. Waiting for OEM to locate material
              - OEM's packed equipment from different systems on the same skid
     b. Ordering multiple skids in hopes of finding a particular piece; taking additional time to shuffle skids
     c. Poor Labeling - Skids, Material etc - In a few cases, the OEM would have to sit there and tell us where particular equipment would     go, they would also relabel material on site by hand in many cases
     d. Different systems being shared. In turn, the problem was pushed down the road.

2. OEM's using lower quality material - Anchors + Other Hardware especially

3. Fabrication Issues with OEM equipment

4. Shipping Damage - On Rail+Other during Ocean Trip

ROOT CAUSE

1. No Contents List, OEM could not tell you the exact contents of each skid
2. OEM's are cheap
3. OEM's moving to quickly
4. Material packaged and secured poorly

COURSE OF ACTION

1. CCC requested additional lay down to shake material out

2. CCC had to order a lot of anchors + hardware

3. 74 change orders

4. CCC took pictures whenever skids was uncovered if damage was found.

LESSON FOR FUTURE PROJECTS

- Document control is very important while working with Korean OEM's from our experience
- Need to have necessary staff to cover all of the change orders + document control + drawing changes
