  STATEMENT OF PROBLEMS
Subpar compressive strength test results for Masterflow 648 CP Plus. CCC did not follow the manufacturer''''''''s placement procedures. CCC did not monitor site conditions at the time of the pour. There was a lack of communication between CCC/Tooles/Wade Trim/SOMAT. In addition, the wrong ASTM test was used.

COURSE OF ACTION
N/a

LESSON LEARNED
Follow manufacturers recommendations and procedures which typically will include a representative from the grout manufacturer to be present at the time of pour. Note specific site conditions at the time of pour (temp humidity etc). Review the type of ASTM test required for the product and make sure the product is stored correctly. Review the test results with the entire team before releasing the project to the GC.
