| STATEMENT OF PROBLEM Concrete pour was cancelled after 20 yards arrived on site. Shake hardener color was incorrect. Color was not specified by the PM when ordering. Superintendent did not verify color of pour until morning of pour. The pre-pour checklist was incomplete. COURSE OF ACTION Pour rescheduled at a later date (cost and schedule impact) LESSON LEARNED Complete prepour checklist. Verify shake hardener color before purchase. List all hardeners, stains and colors on the checklist. |
